Please provide the following information. Please note that all requested information must be completed and all questions must be answered. Additionally, you must provide three letters of recommendation. One of those letters must be from your principal or other supervisor, one of those letters must be from a current or former student, and one of those letters must be from a teaching colleague.
All materials must be received by 11:59 p.m. on April 30, 2026. Do not submit any materials that are not specifically requested.

2. In a single document, please provide your responses to the following five topics (maximum of 500 words per item).
  • Describe ways you have successfully encouraged partnerships with families or community members that have positively impacted student learning.
  • Describe strategies or initiatives you have taken to ensure that students of all backgrounds and abilities are able to thrive.
  • Describe ways you have been able to elevate student learning for your own students and other students through collaborations with your teaching colleagues.
  • Describe a project, program or curriculum you have developed that creates a dynamic learning environment for students to engage with real world problems or situations.
  • Summarize your personal beliefs about what distinguishes a truly outstanding teaching from a competent teacher.
